We have a GP Worker and Hospital Worker funded by NHS Haltons Clinical Commissioning Group (CCG) to help GP practices and hospitals to identify and support carers. Please tell your GP practice that you are a carer and they will note your records so they can better support you. This could be via free Flu vaccinations, referral to our Carers’ Centre for you to be registered and supported and they will be aware your health may be affected by your caring role. Carers are twice as likely to suffer from ill health, compared to non-carers.

If you or the person you care for have to go into hospital, please remind your GP that you are a carer and ensure they liaise with the hospital. Our Hospital Worker can support you if you or the person you care for goes into hospital.

Hospital Card

Carers who are caring for someone in hospital are entitled to open visiting. Carers just need to tell the ward they are a carer and they will issue a card for open visiting.

The card also has our number if you need to register as a carer with us.
